Proctor Nomination Form

Exams are given in the Engineering Outreach Office, 129 Kurt F. Wendt Library, 215 North Randall Avenue, Madison, WI 53706.

Students who cannot take exams at that location may arrange to have exams proctored off-campus. Use the Proctor Nomination Form to choose a person to act as a proctor:

  • the personnel or human resource director at your place of employment
  • a librarian
  • an administrator at a local school or college
  • a member of the clergy (this must be a full-time job, some restrictions apply)

The proctor cannot be related to you in any way or be chiefly a friend (it is not necessary that you know your proctor — many students meet their proctor for the first time when they approach them about proctoring).

Transcript Requests

You may submit transcript requests online, in person on the UW-Madison campus, or by mail. Details are available from the Registrar's Office.
