The Fast Early Feedback forms, located on pages 11 through 19 in the Teaching Assistant Evaluation and Improvement Handbook, help get information about your students, and the Fast Feedback forms help obtain information about how you are doing throughout the semester. Note the type of the TA position at the top of the page. Make copies of the appropriate form as necessary for your class. You can hand out the Fast Feedback form several times before the mid-semester evaluation form is distributed. Don't forget you can always ask your students additional questions.
