The Industrial and Systems Engineering Department requires that all applicants submit an application portfolio as part of the application process. It is the applicant's responsibility to prepare the portfolio and ensure that it is complete before sending it to the ISyE Department.

Sending all necessary documents in a single package will speed up the processing of your application and prevent any unnecessary delay in providing you with a response.

The following materials must be submitted in one package to the degree program at this address:

Madison, WI 53706-1572

  1. Official transcripts of all undergraduate and graduate academic work (must come in an envelope sealed by the institution). International student transcripts should be in the original language accompanied by an official English translation. Documents must be issued by the school with the official seal/stamp and an official signature. Secondary school records are not required.

  2. PhD applicants must also include a resume in this package or upload it to the Graduate Applications website.

If you require a delivery receipt for your application portfolio, please arrange this with your carrier. The department does not provide delivery confirmation. We will enter the materials received into the Graduate Applications website within two weeks of receiving the package.
