Applicants for graduate study in Industrial and Systems Engineering must complete the following steps:

1. Ensure that they meet ALL the requirements for acceptance.
2. Fill out an application form and submit the application fee.
3. Letters of recommendation can be handled one of 3 ways (please only submit one letter from each recommender in only one of the following ways):
   • The preferred method: You can request that your recommenders complete e-versions of the Graduate School's recommendation form as part of your online application. This is preferred since we try to make all of the application material available online for faculty review.
   • You can personally request of your recommenders that they send a completed paper copy of the appropriate Graduate School's online recommendation form to the department address.
     • Recommendation forms for MS applicants
     • Recommendation forms for Phd applicants
   • You can personally request of your recommenders that they send a regular letter via regular mail to the department address.
4. If not paid by credit or debit card during the online application process, please submit the $56 application fee.
   • The application fee can be paid by personal check or money order (please do not send cash) made payable to "University of Wisconsin-Madison" and drawn on a U.S. bank. Please send to this address:

5. Ask ETS to submit your GRE and/or TOEFL scores, if required, to the Graduate School, which makes them available online to all the departments to which you have applied. The institution code, therefore, is the only number needed : 1846.
6. SEND NO PAPER DOCUMENTS TO THE GRAD SCHOOL unless you are admitted and required to provide proof of adequate financial resources.
7. Submit an application portfolio to the ISyE Department. Send all application materials to: ISyE Grad Admissions, UW Madison, 3182 ME Bldg, 1513 University Avenue, Madison, WI 53706-1380.
8. Log on to the Graduate Applications website to track your application’s progress. Shortly after you apply, you should receive a welcome email from this website. Please also upload your statement of purpose at this site if you did not submit it with your application.
9. Application deadlines for admission to the ISyE graduate program:

   FALL
   Application deadline (both MS and PhD): March 1
   Financial support deadline to be considered for a graduate school fellowship: December 1
   Other financial support consideration deadline: February 1

   SPRING
   Application deadline: October 1
